The daily irritant: Filling out those proxy forms

This is yet another challenge for me whenever I am required to complete forms that are in hard copy format.  I guess that this is a challenge that will probably be forever around.

I need sighted assistance to do this.  I need someone to read the forms to me and then I need them to show me where to sign them.

Too much of society still needs to understand that forms in hard copy deprive me of my independence and privacy and these are two basic rights that everyone is entitled to.
